  2. Форма 183 нужна только для студентов, то есть для тех, кто диплом не получил.

  3. Про оплату кажется да. Я тоже второй раз регистрировалась, когда была готова оплатить и когда открыли мой eligibility period.

скажите, во второй раз регистрируясь, Вы что отвечали “Yes, I have previously submitted an application to ECFMG for examination, even if I did not take the examination.” Или начинали всё заново?

я перевела диплом сама, в бюро его просто проверят. Мне уже переводили его, я его уже и нотариально заверяла. Но без вкладышей. Так что проблем с самим переводом быть не должно. Но вот документ ТОЛЬКО НА АНГЛИЙСКОМ в Москве сделать сложно. Особенно это касается нотариуса. Если с бюро, я так поняла, можно договориться, то нотариус - большой вопрос. Кто-нибудь переводил и заверял диплом в Москве? можете подсказать где это оформят правильно?

Не пойму зачем вам нотариус и бюро переводов одновременно? Я просто перевела диплом, поставили мне туда все что требуется. У нотариуса ничего не заверяла. Все прошло.

Если честно, то я вообще уже плохо помню:) Я регистрировалась год назад, дали мне ID и пароль. Мне на почту все время новости приходили от ECFMG. Когда зашла второй раз то пароль и ID остались те же, но мне пришлось заново заполнять application и после этого оплатила. Но честно говоря вспомнить точно не могу.

Here are the steps that you have to follow when registering for the first time with ECFMG in order to apply for USMLE.

First of all check if your medical school is listed in IMED, if it’s not you should inform your medical school immediately. To check the IMED list go here https://imed.faimer.org/

1- USMLE ID
The first step ever is to obtain a USMLE Identification Number also called USMLE ID or ECFMG ID this is done by going to this web page https://secure2.ecfmg.org/usmleidreq...navigator.aspx
After you enter your information such as name, email, address, etc they will send you an email giving you your ECFMG ID (8 digits number) and a password to access the online services.
Wait one week and if they don’t send you the email (check your junk folder also) then call them +1 (215) 386 5900.
This USMLE ID is yours forever and you should record it in a safe place (not on your Kaplan lecture notes!)

2- IWA application
Before logging in to IWA make sure of the following:
Your Internet access is live and continuous (electricity or Internet outage may prove disastrous)
Your printer is ready and has enough papers.
Using your ECFMG ID and password you can log in to IWA and begin the application process. You can do that by going to this page https://iwa2.ecfmg.org
After logging in you will be asked to fill out online forms (be very careful while filling these) and if for any reason you are stumbled upon a question that you don’t know the answer immediately, you can save the application and log out and come back to at a later time. Then you will be asked to print out some forms that you have to fill by hand and then send by mail to ECFMG.

3- Forms
Form 344 is a check list of what documents you have to attach.
Form 345 is medical school release request (basically you are allowing ECFMG to obtain your information from your medical school). Two copies needed of this form.
Form 183 or Form 186 these are forms that let ECFMG make sure that you (your name and a your photo) are in fact existent and that this is your signature and this is your ECFMG ID. These forms may have to be signed by your medical school official and/or by a notary public, for detailed information about these forms please read The Complete Guide to ECFMG Forms

4- Documents
The documents that you will be sending with these forms are
Medical school diploma + translated if it is not in English
Two passport sized photographs
Some other documents might be needed (like for example if you have transferred credits)
5- Send the forms and documents
Now that you collected and filled all these documents and forms you should send them to ECFMG Philadelphia office (address will be given to you in the forms and during the online application).

6- Wait for confirmation
ECFMG will tell you by email and by mail that they have received your forms and documents then they will send forms to your medical school asking them to verify your medical school credentials and also requesting your final medical school transcript (if you are a graduate) from them.

7- ECFMG to Medical School Forms
These forms you do not have access to it and you are not allowed to send them to your school, The ECFMG will send them.
They will send one copy of your 345 to tell the school that you allowed them to ask about you.
They will also send your Diploma (that you initially sent to them see step 4 above) and they’ll ask the school to authenticate it.
They will also send a form called (Verification of Medical Education) the questions in that form are these;
what was the requirements before you entered the school
how many weeks of medical education you completed
what degree was awarded (if you are a graduate)
asking to verifying the attached diploma
dean or other authorized registration officer’s signature, date, and name
any interruptions or extensions in your medical education
any disciplinary probation you had or currently have
any professional misconduct that you have during your school years
any negative reports about you in the school
any limitations or special requirements by you
8- Medical School to ECFMG
Your medical school would have to respond to these forms and should also send your final medical school transcript (in English, if not you will be asked to pay for the translation fees).
The last two steps may take several months! However, if your school participate in the ECFMG Medical School Web Portal then it may take only few days.

9- OASIS
After waiting for steps 7 and 8 above to finish you are now ready to check your status by logging in to OASIS (using your same ECFMG ID and password). The link is this ECFMG On-line Services
When logged in look at the left hand side panel and click on “Medical Education Credentials and Standard ECFMG Certificate” and the status that you should be happy with is this “Medical education credentials received and verified”, any other status means that you’re not done yet.

10- Apply
Once you received that status you can now apply for any step you like using IWA or OASIS.
The fees for the exam has to be paid at this point. Current fee requirements are listed in this web page ECFMG | Selected Fees
Note that you are applying for an eligibility period not for exact exam date.
The eligibility periods are three months for all exams except CS it’s 12 months.

11- Scheduling Permit
When you finish applying online you’ll get a scheduling permit with permission confirmation number and exam registration ID that you should print out and take it with you to your exam. (note there is no more orange slips that you have to wait for in the mail as before).

12- Prometric
After receiving a confirmation email and mail from ECFMG for your application status (Scheduling Permit) you can then go to Prometric website and choose your exam date (of course within your eligibility period).

These steps are general and there might be significant variations depending on your special circumstances.
Be very careful while filling any form online or offline as they will held you responsible for any little thing you say and it’s very difficult to change thereafter.
Processing times varies with the time of the year and on how fast your medical school responds to ECFMG.

Forms 183 and 186

In order to obtain a Certification Statement (Form 183) or Certification of Identification Form (Form 186), you must first complete the on-line part of an exam application using ECFMG’s Interactive Web Applications (IWA).

The Certification Statement (Form 183) is the paper certification required by ECFMG for applicants who are students enrolled in medical schools that do not participate in the ECFMG Medical School Web Portal (EMSWP). The Certification of Identification Form (Form 186) is the paper certification required by ECFMG for applicants who are medical school graduates or are students enrolled in medical schools that participate in EMSWP.

After you have completed the on-line part of your exam application, IWA will automatically generate the appropriate form (Certification Statement or Certification of Identification Form). You will then be able to print the form through IWA.

If you have already completed the on-line part of an IWA exam application and need to reprint your Certification Statement or Certification of Identification Form, access IWA.

Overview: Application for USMLE Step 1, Step 2 CK, and Step 2 CS

 You may use IWA to apply for the United States Medical Licensing Examination (USMLE) Step 1, Step 2 Clinical Knowledge (CK), and Step 2 Clinical Skills (CS). You may use IWA to apply for examination regardless of whether you are applying to ECFMG for examination for the first time or have applied for examination previously. ECFMG will begin processing your exam application after receiving the on-line part of the application; either the Certification of Identification Form (Form 186), if required, or the Certification Statement (Form 183); and any other required documents.

IMPORTANT NOTE: The Application for ECFMG Certification became a requirement for ECFMG Certification on September 15, 2010. Effective on this date, international medical students/graduates must submit an Application for ECFMG Certification before they can apply to ECFMG for examination. TO COMPLETE AN EXAM APPLICATION, YOU WILL NEED:

OR

Before you can begin the exam application process, you must have your USMLE/ECFMG Identification Number and on-line services password. You must also have completed the Application for ECFMG Certification. You can check the status of your Application for ECFMG Certification using OASIS.

If you have never submitted an application to ECFMG, you will need to complete the ECFMG On-line Authentication Process to obtain a USMLE/ECFMG Identification Number. Once you have received the Identification Number and temporary password via e-mail, you can use them to log into IWA and complete the Application for ECFMG Certification. Once your Application for ECFMG Certification has been submitted, you can begin an application for examination.

If you have previously submitted an application to ECFMG, you will need your USMLE/ECFMG Identification Number and password. If you have used ECFMG’s On-Line Applicant Status and Information System (OASIS), you already have a password. If you have a USMLE/ECFMG Identification Number but do not have a password, you must complete ECFMG’s On-Line Authentication Process and wait for a temporary password to be e-mailed to you before you can proceed with an IWA application. If you forgot your USMLE/ECFMG Identification Number, you must complete the ECFMG On-line Authentication Process to obtain your number. Click here to go to ECFMG’s On-Line Authentication Process. ECFMG will perform a thorough search using the information you provide and, if a USMLE/ECFMG Identification Number exists for you, your ID number will be sent to your e-mail address of record. If the system is unable to identify you, your information will be submitted to ECFMG for processing. Before you begin an exam application, you must read the editions of the ECFMG Information Booklet and the USMLE Bulletin of Information that pertain to the eligibility period in which you take the exam. Before applying for Step 1 or Step 2 CK, you should also be familiar with the Biometric Enabled Check In System Privacy Notice for USMLE Candidates, which describes the privacy and security practices that have been established for the Biometric Enabled Check In System (BECS) program implemented by Prometric as part of standard test day procedures for computer-based Step examinations.

You should gather the medical education information needed to complete the application. This information includes the names and addresses of medical schools you have attended or are attending, attendance dates, clinical clerkships completed, and any credits that you transferred to the medical school that awarded or will award your medical degree.

If you are completing an exam application for the first time, you should allow approximately one hour to complete all items. You should be able to complete subsequent applications in a shorter period of time since you will not need to re-enter all of the information.

You should also review the Frequently Asked Questions (FAQs) before you begin an exam application.  There are two parts to the exam application.

The on-line part of the application

AND

Either the Certification of Identification Form (Form 186) OR the Certification Statement (Form 183).
ECFMG must receive both the on-line part and the Certification of Identification Form (Form 186), if required, or the Certification Statement (Form 183) in order to process your application.

  1. On-line Part

You must begin with the on-line part, which includes most of the application items. Instructions for the application items are available throughout the process. You can also print the application instructions for all items at any time.

When you begin an on-line application, your application will be assigned an Application Identification Code. (The Application Identification Code is different from, and should not be confused with, your USMLE/ECFMG Identification Number.) You should write down the Application Identification Code and keep it in a safe place.

As you move from screen to screen, the information you provide will be saved. If you are unable to answer all application items in one session or you are disconnected from the application, you can return to it later by clicking the link to your saved application after logging into IWA.

Although you do not have to answer all items in one session, you must complete all on-line application items within 14 days of the date you last saved the application. Otherwise, the application will no longer be valid, and you must start another application.

After completing the application items for one exam, you will have the opportunity to apply for other exams for which you may be eligible on the same application.

When you have completed the on-line part of the application, you will have the opportunity to review all information for accuracy and make any necessary corrections or changes before submitting it. As the last step in the on-line part of the application, you will print your Certification of Identification Form (Form 186), if required, or Certification Statement (Form 183).

  1. Certification Form

Medical School Graduates: You will be required to complete a Certification of Identification Form (Form 186) if you do not have a valid Certification of Identification Form (Form 186) on file with ECFMG. The Certification of Identification Form (Form 186) must be completed by you and an authorized official of your medical school and/or a Consular Official, First Class Magistrate, Notary Public, or Commissioner of Oaths. The on-line part of your application will be transmitted to ECFMG only when you click on one of the links to proceed to the Certification of Identification Form (Form 186).

There are three Certification of Identification Form (Form 186) options for medical school graduates:

“Certification of Identification Form (Form 186) - Certification by Medical School Official,” for all medical school graduates
“Certification of Identification Form (Form 186) - Certification by Notary Public,” for graduates of medical schools that participate in the ECFMG Medical School Web Portal (EMSWP) and for graduates whose medical education credentials have been primary-source verified by ECFMG

Additionally, ECFMG must receive the Certification of Identification Form (Form 186) and any additional documents and photographs as soon as possible but no later than 12 months from the date you submit the on-line part of your application. ECFMG must receive the on-line part of your application, the Certification of Identification Form (Form 186), and all required documents to process your application.

Your application will not be considered complete until the completed Form 186 (if you do not have a valid Form 186 on file with ECFMG) and any other required documents are received at ECFMG.

If you are a graduate of a medical school that participates in EMSWP and your medical education credentials have not been primary-source verified by ECFMG, ECFMG cannot complete the processing of your application until your medical school verifies your status through EMSWP. ECFMG will make your record available to your medical school and request verification typically within three weeks of receipt of a complete application. Step 1/Step 2 CK Eligibility Periods

When you apply for Step 1 or Step 2 CK, you must select a three-month period, such as January-February-March, February-March-April, etc., during which you would like to take the exam. This three-month period is referred to as your eligibility period. If you apply for Step 1 and Step 2 CK at the same time, you can select the same eligibility period for both Step 1 and Step 2 CK, or you can select a different eligibility period for each exam. You must take the exam during the eligibility period assigned to you. You can take the exam on any day that it is offered during your assigned eligibility period, provided there is space available at the test center that you choose.

When choosing your eligibility period, remember that: Prometric test centers are closed on major local holidays; Step 1 and Step 2 CK are not offered during the first 14 days of January; and the busiest testing times in the United States/Canada testing region are May through July and November through December. Your eligibility period will not be adjusted to compensate for dates when the exams are not available.

To obtain a specific eligibility period:

ECFMG must receive your complete exam application by the 24th day of that eligibility period, and
The processing of your application must be completed by ECFMG by the 25th day of that eligibility period.
Example: On May 3, an applicant selects the May-June-July eligibility period for Step 1 and submits her application. The processing of the application is completed by ECFMG on May 23. The applicant is assigned the May-June-July eligibility period that she selected on her application.
Keep in mind that delays or interruptions in postal service may delay delivery to ECFMG of materials needed to complete your application.

If you are a medical school graduate whose medical education credentials have been primary-source verified by ECFMG; a graduate of a medical school that does not participate in EMSWP and your medical education credentials have not been primary-source verified by ECFMG; or a student enrolled in a medical school that does not participate in EMSWP, it typically takes three weeks from receipt of a complete exam application for ECFMG to complete the processing of your application.

If you are a graduate of a medical school that participates in EMSWP and your medical education credentials have not been primary-source verified by ECFMG or if you are a student enrolled in a medical school that participates in EMSWP, ECFMG cannot complete the processing of your exam application until your medical school verifies your graduate/student status through EMSWP. ECFMG will make your record available to your medical school and request verification typically within three weeks of receipt of a complete application. The time needed to complete the processing of your application is contingent upon your medical school’s response to the EMSWP verification request.

For all applicants, although you can select an eligibility period as late as the 24th day of that eligibility period, there is no guarantee that you will be assigned that eligibility period, since the processing of your application may not be completed by the 25th day of the eligibility period. If the processing of your application is not completed in time to assign the eligibility period you select, you will be assigned to the next available eligibility period, based on the date your application is processed. The eligibility period assigned to you will be listed on your scheduling permit. You must read the editions of the ECFMG Information Booklet and the USMLE Bulletin of Information that pertain to the eligibility period in which you take the exam.

Example: On July 23, an applicant selects the July-August-September eligibility period for Step 2 CK and submits his application. The processing of the application is completed by ECFMG on August 12. The applicant is assigned the next eligibility period, August-September-October.
As shown in both examples above, you will be able to obtain an eligibility period that has already begun, or that will have begun by the time you are registered and your scheduling permit is issued. Your assigned eligibility period will not be adjusted to compensate for days that have already passed.

Eligibility Period Extension
If you are unable to take Step 1/Step 2 CK during the three-month eligibility period assigned to you, you may request a one-time extension. You may extend your eligibility period only through the next, contiguous eligibility period (the next eligibility period that does not overlap with your original eligibility period).

Example: An applicant’s original eligibility period is April-May-June 2012. The eligibility period may be extended only through July-August-September 2012.
You may request an extension on-line using IWA. If you take the exam during your original eligibility period or if you do not take the exam, you will not receive a refund or credit of the eligibility period extension fee.

You may request an extension of your eligibility period only once for each exam registration. You may request an extension of your eligibility period only after your eligibility period has begun. The processing of your request must be completed by the published deadline or your request will be rejected. For specific information on deadlines, refer to IWA. If you are a graduate of a medical school that participates in EMSWP and your medical education credentials have not been primary-source verified by ECFMG or if you are a student enrolled in a medical school that participates in EMSWP, ECFMG cannot complete the processing of your request until your medical school verifies your graduate/student status through EMSWP.

Additionally, you must continue to be eligible to take the exam during the extended eligibility period (you must still be officially enrolled in a medical school located outside the United States and Canada that is listed in IMED and the “Graduation Years” for your medical school must be listed as “Current,” or you must be a graduate of such a medical school and your graduation year must be included in the school’s IMED listing). If your eligibility period is extended, NBME will issue a revised scheduling permit reflecting the extension. You must access this revised scheduling permit using IWA and bring it to the test center on the date of your exam.

If you have a scheduled appointment during your original eligibility period and need to cancel and reschedule the appointment for your extended eligibility period, you must contact Prometric to cancel or reschedule that appointment. Requesting an extension of your eligibility period does not cancel a scheduled appointment. A fee may be charged if you change your appointment, depending upon how much notice you provide when making the change. See Rescheduling in Scheduling the Examination.

If you are unable to take the exam during your original or extended eligibility period or you are unable to extend your original eligibility period, you must reapply, including payment of the appropriate fee(s), to take the exam.

Как можно поменять инфо о medical school в своём аккаунте на ecfmg (IWA). Зарегился там пару лет назад. Скоро собираюсь регистрировать на стэп 1, но не могу посмотреть инфо, которое я указал при регистрации тогда. Хочу удостовериться, что всё инфо указано верно, а если нет, - отредактировать. Но никак не могу найти, как это сделать.

  1. Access and Ability to Correct Personal Data

Using OASIS, properly identified applicants for ECFMG Certification and ECFMG-certified physicians may access and verify or change such personal information as name of record, postal address, e-mail address, fax number and status of their financial account. You may also check the status of your USMLE applications and score reports, USMLE scheduling permits, and medical education credentials. In order to add to or change information in OASIS, you must have both your USMLE/ECFMG Identification Number and a password. If you do not have a password, you may obtain one by accessing OASIS and following the instructions for establishing a new account. OASIS is available 24 hours a day, 7 days a week, with the exception of maintenance periods.

Upon a properly authenticated request via postal mail, ECFMG will provide Users who are applicants for ECFMG Certification or ECFMG-certified physicians access to personal information that cannot be otherwise accessed through OASIS, such as credit card information, social security and/or national identification numbers, which User may then change or update. To request access to your personal information, you may write to us at the address listed below in “Inquiries and Concerns.”

Users of EMSWP can request changes to their personal information by contacting their medical school’s EMSWP Coordinator. The EMSWP Coordinator can request such changes for all Users at the medical school by accessing EMSWP at https://emswp.ecfmg.org. The medical school’s EMSWP Coordinator can also request such changes by contacting ECFMG’s EMSWP Administrator by e-mail at emswpadmin@ecfmg.org. ECFMG’s EMSWP Administrator will review these requests and, if approved, implement the requested changes.

Users of EVNet can make changes to their contact information through EVNet.

Applicants for employment may update their information through the on-line employment application at Employment Application - ECFMG Jobs.

Individuals participating as advisors in IAN may update certain information in their Advisor Profiles by accessing IAN at ECFMG On-line Services.

Individuals participating in IAN as advisees may update certain information in their profiles by accessing IAN at ECFMG On-line Services.

Individuals requesting return of their ERAS documents through RODS can update information related to their request by accessing the RODS feature of OASIS prior to submitting their request.

Before applying for an eligibility period in 2012, you must have read the ECFMG 2012 Information Booklet and the USMLE 2012 Bulletin of Information. If the processing of your application is not completed in time to assign the eligibility period you select, you will be assigned to the next available eligibility period, based on the date your application is processed. If the next eligibility period extends into 2013 and you test in 2013, you must become familiar with and will be subject to the policies and procedures detailed in the ECFMG 2013 Information Booklet and USMLE 2013 Bulletin of Information.

Достоверный ответ я получил от ECFMG: Please be advised that ECFMG® has no medical school information on your permanent file, therefore you may enter the correct information when submitting an application for examination.
Так что, данные о мед скул поменять невозможно, их просто нет в permanent file.
