Послезавтра выезжаем семьёй в Киев. Голова идёт кругом. Все документы, фотографии, прививки и выписки сделаны. Нужно всё собрать “до кучи”. Нас двое, и двое детей. Ехать будем машиной. Вся эта подготовка немного сбивает с толку, переживания переполняют. Боюсь что-то пропустить, что-то забыть дома, забыть распечатать и т.п. Все официальные сайты проштудированы многократно, сделал себе своего рода чеклист для медкомиссии и посольства. Но всё-равно “мандражирую”
Не забыл ли я чего, дорогие форумчане?
Pre-Interview checklist:
□ A copy of your NVC interview letter (does not apply to Diversity Visa, fiancé(e), adoptive, or asylee/refugee applicants).
□ Unexpired passport valid for six (6) months beyond your intended date of entry to the United States. Passport must be registered for delivery by the courier service company TMM- Express. Please see passport registration instructions below.
□ Two (2) color photographs of each person applying for a visa (5 cm x 5 cm).
□ Confirmation page from the Form DS-260 Application for an Immigrant Visa you submitted online at Immigrant Visa - Technical Specification Information Page.
□ Your original birth certificate, English translation (only if this document is not in Ukrainian or Russian languages), and a photocopy.
□ Medical examination results in a sealed envelope (if the physician gives you these results).
□ Original or certified copies of birth certificates for all children of the principal applicant (even if he or she is not accompanying).
□ If you are married: Your original marriage certificate and a photocopy.
□ If you are older than 16 years of age: The original police certificate from your country of current residence and countries where you resided for more than one year after the age of 16. If you are Ukrainian citizen or resident, you must present a Ukrainian police certificate issued within the last one year. Police certificates from Crimea issued by the Russian Government will NOT be accepted for visa purposes.
□ If you have served in any country’s military: Military records and a photocopy.
□ Documents showing that you have either a qualifying high school education OR have two years of qualifying work experience in the last five years immediately prior to application (for the principal applicant only; more information is available online).
□ Payment in cash of the $330 Diversity Visa Application Fee.
